You can find Form 1095-A in your MNsure account on the Notifications page and also in your Enrollment Dashboard.
1. Sign in to your MNsure account and click the Your Account link at the top of the page. This will take you to your account homepage.
2. Select Notifications from the top menu bar. If a 1095-A form is available, it will be here.
3. Click the small arrow next to the notice to open the notification for more information.
4. You’ll see an attachment link for the 1095-A. Click the link to download the form.

2. Click View Eligibility Results.
Your Health Care Results page will load.
3. Click the Enroll in Health Plans button to go to your Enrollment Dashboard.
Your dashboard will load.
4. Select “My Inbox” from the dashboard menu to open your inbox and view your notices. If a 1095-A form is available, it will be here.
