Documents to Verify Your Life Event

Send a copy of at least one of the documents below: 

• The court order defining the adoption, foster care or placement for adoption or foster care
• Court order defining child support 
• Other court order

Documents must show that you adopted, had a foster care placement or were ordered by the court to provide coverage in the past 60 days. The documents must include your name, the dependent’s name and the effective date of the action.

How to send your documents.

If your tribal membership has not already been verified with MNsure, send a copy of at least one of these documents:

• Tribal affiliation card
• Tribal affiliation document
• A certified letter or membership list from the tribe will be accepted if you do not have either one of the above documents

Documents must show the member's name and name of the tribe.

How to send your documents.

Send a copy of the birth certificate. The birth certificate must show:

• The child's name and birth date.
• The birth date occurred in the last 60 days.

Documentation may not be required if MNsure can obtain this information through federal data. You can call us at 651-539-2099 (or 855-366-7873 outside the Twin Cities) to check if verification is needed.

How to send your documents.

Send a copy of at least one of the documents below:

• Court order with the date of divorce or legal separation
• Death certificate 
• Public notice of death (example: published obituary)

Documents must show that a qualified health plan enrollee lost a dependent or is no longer a dependent due to death or divorce in the past 60 days. These documents must include the enrollee’s name and the date of death, divorce or legal separation.

How to send your documents.

Send a copy of at least one of the documents below:

• Birth certificate
• Certification of report of birth
• Certification of citizenship or naturalization
• Evidence of U.S. Civil Services employment before June 1, 1976
• Federal or state census record
• Final Adoption decree
• Machine-readable immigrant visa on unexpired foreign passport
• Medical records
• Military record showing U.S. place of birth
• Minnesota enhanced driver’s license or enhanced identification card
• Northern Marianas ID card
• Official religious records
• PASS card
• Report of birth abroad of a U.S. citizen
• School records
• Section 1-1 of the Child Citizenship Act of 2000 documentation
• Temporary I-551 Stamp
• Tribal Affiliation card or document
• Unexpired foreign passport
• U.S. citizen ID card
• U.S. passport
• DS2019 (certification of eligibility for exchange visitor status)
• I-20 (certification of eligibility for non-immigrant student status)
• I-94 (arrival/departure record)
• I-94 (arrival/departure record) in unexpired foreign passport

• I-571 (refugee travel document)
• I-688 (temporary resident card)

• Other documents generally accepted as legal proof of citizenship or immigration status such as:
  • Administrative order staying removal issued by the Department of Homeland Security
  • Document indicating withholding of removal
  • Form I-360 prima facie determinations
  • Office of Refugee Resettlement

Documents must show you gained citizenship or lawfully present status within the past 60 days. These documents must include your name, immigration status and date on which citizenship or lawful presence was gained.

How to send your documents.

Send a copy of at least one of the documents below: 

• Letter from health insurance company that indicates the type of coverage lost and the last date of coverage
• Letter from employer that indicates the type of coverage lost and the last date of coverage
• Employer plan notice that indicates the type of coverage lost and the last date of coverage

Documents must show that you lost qualifying health coverage in the past 60 days or that you will lose coverage in the next 60 days. These documents must include your name, the coverage type and the last day of coverage. 

If documents indicate a voluntary termination or term for non-payment of premiums, SEP will be denied.

How to send your documents.

Send a copy of at least one of the documents below:

• Letter from health insurance company that indicates loss of coverage
• Notice of loss of benefits from a state or federal government program such as Medical Assistance, MinnesotaCare, Medicare or TRICARE.

Documents must show that you lost qualifying health coverage in the past 60 days or that you will lose coverage in the next 60 days. These documents must include your name, the coverage type and the last day of coverage.

Documentation may not be required if MNsure already has information about the loss of your Medical Assistance or MinnesotaCare coverage. You can call us at 651-539-2099 (or 855-366-7873 outside the Twin Cities) to check if verification is needed.

How to send your documents.

1. Send a copy of the marriage certificate: 

• The marriage certificate must show your name and the date of marriage.
• The date of marriage must be within the past 60 days.

2. Send also a copy of one of the following documents that prove at least one spouse had health care coverage prior to the date of marriage (*see exceptions):

• HIPAA certification showing coverage within 60 days of the date of marriage
• Pay stub documenting employer/insurance deduction within 60 days of the date of marriage
• Documentation of premiums paid or invoice within 60 days of the date of marriage
• Coverage closure or termination letter showing the last date of coverage within 60 days of the date marriage
• Other documentation showing that the last date of coverage was within 60 days of the date of marriage

*Exceptions: Prior coverage does not need to be proved if one or more of the following conditions apply to at least one of the spouses:

• Enrolled in coverage through MNsure
• Member of a federally recognized tribe
• Lived in a foreign country, or a United States territory, or a service area where no qualified health plans were available, for one or more days during the 60 days prior to the date of the move: send a signed written statement under penalty of perjury stating one of these reasons for why you did not have health care coverage prior to the move.

Documents must show that you were married in the past 60 days. These documents must include your name, date of marriage and a date of prior coverage (if proving prior coverage applies to you).

How to send your documents.

Send documents to prove the date you permanently moved to Minnesota was within the past 60 days. You may also need to send documents to prove you were enrolled in health care coverage prior to the move.

1. Send a copy of at least one of these documents to prove the date of the move:

• Copy of current lease
• Copy of current mortgage or real estate transition
• Recently issued Minnesota driver’s license or state ID
• Recent utility bill or billing statement from the new address
• Recently issued voter registration card
• Regarding a school-aged child, school records showing recent enrollment in school with new address

2. Send also a copy of at least one of the following documents to show your previous address:

• Copy of previous lease
• Copy of previous mortgage or real estate transition
• Previous state driver’s license or state ID
• Utility bill or billing statement from the previous address
• Voter registration card from previous address
• Regarding a school-aged child, school records showing recent enrollment in school with previous address

3. You may also need to send a copy of at least one of the following documents to show you had health care coverage prior to the move (*see exceptions):

• HIPAA certification showing coverage within 60 days of the move
• Pay stub documenting employer/insurance deduction within 60 days of the move
• Documentation of premiums paid or invoice within 60 days of the move
• Coverage closure or termination letter showing the last date of coverage within 60 days of the move
• Other documentation showing that the last date of coverage was within 60 days of the move

*Exceptions: you do not need to prove prior coverage if one or more of the following applies to at least one of the members of your household seeking to enroll:

• Enrolled in coverage through MNsure
• Member of a federally recognized tribe
• Lived in a foreign country, or a United States territory, or a service area where no qualified health plans were available, for one or more days during the 60 days prior to the date of the move: send a signed written statement under penalty of perjury stating one of these reasons for why you did not have health care coverage prior to the move.

These documents must include your name, date of permanent move and a date of prior coverage (if proving prior coverage applies to you).

How to send your documents.

Send documentation to prove the effective date of the individual coverage health reimbursement arrangement (ICHRA) or qualified small employer health reimbursement arrangement (QSEHRA). 

Send a copy of the written notice from your employer explaining the terms of the HRA offered. the document must include:

• An offer for a qualifying HRA that took effect in the past 60 days or that will take effect in the next 60 days. 
• Your name, the HRA type and the start date of the benefit.

How to send your documents.

Send release documents from the department of corrections or correctional facility showing your name and release date.

Documents must show that you were released from incarceration within the past 60 days.

How to send your documents.

Send documentation that proves the date of the move to a new county or ZIP code within Minnesota. You may also need documents to prove you were enrolled in health coverage prior to the move.

1. Send a copy of at least one of these documents to prove the date of the move: 

• Copy of current lease
• Copy of current mortgage or real estate transition
• Recently issued Minnesota driver’s license or state ID
• Recent utility bill or billing statement from the new address
• Recently issued voter registration card
• Regarding a school-aged child, school records showing recent enrollment in school with new address

2. Send also a copy of at least one of the following documents to show the previous address:
(Note: current MNsure enrollees do not need to send proof of previous address.)

• Copy of previous lease
• Copy of previous mortgage or real estate transition
• Previous state driver’s license or state ID
• Utility bill or billing statement from the previous address
• Voter registration card from previous address
• Regarding a school-aged child, school records showing recent enrollment in school with previous address

3. Send also a copy of at least one of the following documents to show you had health care coverage prior to the move (*see exceptions):

• HIPAA certification showing coverage within 60 days of the move
• Pay stub documenting employer/insurance deduction within 60 days of the move
• Documentation of premiums paid or invoice within 60 days of the move
• Coverage closure or termination letter showing the last date of coverage within 60 days of the move
• Other documentation showing that the last date of coverage was within 60 days of the move
