Send a copy of at least one of the documents below:
Documents must show that you adopted, had a foster care placement or were ordered by the court to provide coverage in the past 60 days. The documents must include your name, the dependent’s name and the effective date of the action.
If your tribal membership has not already been verified with MNsure, send a copy of at least one of these documents:

Documents must show that a qualified health plan enrollee lost a dependent or is no longer a dependent due to death or divorce in the past 60 days. These documents must include the enrollee’s name and the date of death, divorce or legal separation.

MNsure requires documentation to verify most qualifying life events before plan selection is available. Coverage start dates are determined by the type of qualifying life event reported and the plan selection date.
