Your Health Insurance Card

When you sign up for a private health plan and pay your first monthly bill, your insurance company will send you a packet in the mail. This packet will include your insurance card and a summary of benefits and coverage.

You can see a sample card (), but your card may look different. The name of the insurance company is at the top of the card, and most cards include information including:

  1. Your name
  2. Your member number (used so providers know how to bill your plan)
  3. Your group number
  4. Your co-pay amount (what you pay when you get care)
  5. A phone number to call for help
  6. Your plan type (like HMO or PPO)
  7. Your prescription co-pays

Always bring your insurance card when you go to the doctor or the pharmacy.
