Your Health Insurance Card
When you sign up for a private health plan and pay your first monthly bill, your insurance company will send you a packet in the mail. This packet will include your insurance card and a summary of benefits and coverage.
You can see a sample card (), but your card may look different. The name of the insurance company is at the top of the card, and most cards include information including:
- Your name
- Your member number (used so providers know how to bill your plan)
- Your group number
- Your co-pay amount (what you pay when you get care)
- A phone number to call for help
- Your plan type (like HMO or PPO)
- Your prescription co-pays
Always bring your insurance card when you go to the doctor or the pharmacy.
