You may qualify for a special enrollment period (SEP) if you made a permanent move to Minnesota from another state, outside the U.S. or from a U.S. territory. You must have already moved to apply for this SEP.
Please note: Making a temporary move or a move for purposes of receiving medical treatment will not qualify for a SEP.
Key Dates
- 60-day SEP begins: Date of move.
- When you can enroll: Any time during the 60-day window of your SEP once your qualifying life event is verified.
New customers: To avoid a gap in coverage you can create an account and apply at any time, even before your SEP window starts.
- When your coverage starts: The first day of the month after your plan selection date.
Verification Documents
Be prepared to send documentation that proves you made a permanent move to Minnesota within the past 60 days. You may also need to send documents to prove you were enrolled in health care coverage prior to the move. You'll need:
A copy of at least one of these documents to prove the date of the move:
- Current lease
- Current mortgage or real estate transition
- Recent utility bill or billing statement from the new address
- Recently issued Minnesota driver’s license or state ID
- Recently issued voter registration card
- Regarding a school-aged child, school records showing recent enrollment in school with new address
A copy of at least one of the following documents to show your previous address:
- Previous lease
- Previous mortgage or real estate transition
- Previous state driver’s license or state ID
- Regarding a school-aged child, school records showing recent enrollment in school with previous address
- Utility bill or billing statement from the previous address
- Voter registration card from previous address
A copy of at least one of the following documents to show you had health care coverage prior to the move (*see exceptions):
- Coverage closure or termination letter showing the last date of coverage within 60 days of the move
- Documentation of premiums paid or invoice within 60 days of the move
- HIPAA certification showing coverage within 60 days of the move
- Pay stub documenting employer/insurance deduction within 60 days of the move
- Other documentation showing that the last date of coverage was within 60 days of the move
*Exceptions: you do not need to prove prior coverage if one or more of the following applies to at least one of the members of your household seeking to enroll:
- Enrolled in coverage through MNsure
- Member of a federally recognized tribe
- Lived in a foreign country, or a United States territory, or a service area where no qualified health plans were available, for one or more days during the 60 days prior to the date of the move: send a signed written statement under penalty of perjury stating one of these reasons for why you did not have health care coverage prior to the move.
Documents must include your name, date of permanent move and a date of prior coverage (if proving prior coverage applies to you).
How and when do I send documents?
During the SEP application process you'll be instructed on how to send verifying documents, if they are needed. MNsure must receive your documents and have enough time to process them for you to have time to select a plan before your 60-day SEP window expires.
